In the May mailing, members will find a new equal opportunities monitoring form which has been updated to include new disability legislation. We need this information to plan and monitor the services we provide and to meet our obligations under legislation.

The form takes only a few seconds to complete. Even if you have recently filled in an equal opportunities form, please send back this new form in the pre-paid envelope.

If you don't wish to share some or all of this information, please do not throw the form away but return it marked 'information withheld' in some or all categories.

The return rate for these forms has been very poor and seriously reduces the value of the data to your association. Please don't delay.
